The All-you-need-to-know-about-continuity-of-carer newsletter

Description Researcher Development Fund 2022-2023:

The key component of the Scottish’s National Health Service Best Start Government 2017 maternity plan is Continuity of Midwife Care (CMC). Despite the evidence that this model of care is superior in terms of health outcomes for mothers and their families, resources should be provided to affect reasoning and beliefs of midwives to actively embrace the operational implementation of CMC. The ENU midwifery subject group and NHS Lothian researchers have published several papers about CMC, recommending that ground-up change needs to be actively supported. Knowledge is one of the facilitators for the ongoing implementation of the CMC model by the (future) midwifery workforce. The flow and dissemination of knowledge should be offered to (student)midwives on a regular basis, short messages and easy to comprehend including a pragmatic aspect – applied evidence. This can be achieved by an online freely available resource that is easily accessible for (student)midwives to find important and relevant knowledge about CMC. A newsletter is user-friendly and effective wat to directly communicate with the target audience. (ENU) Researchers who like to share their knowledge, can use the newsletter to disseminate the latest (published) evidence.

Developing a midwifery continuity of care research group
2023 - 2024

Description Research Culture Fund 2023/24:

The key component of the Scottish’s Best Start Government 2017 maternity plan is Continuity of Midwife Care (CMC). Although the topic of CMC being embedded in the midwifery curriculum, we would like to shift focus to a research agenda within the midwifery subject group. We are a group of active researchers, in various stages of research careers, with a shared interest and passion for CMC with individual publication output but have little in the way of a shared CMC strategy or plan. This project offers an opportunity to instigate a group leading CMC research and tie this project to the wider school and university agendas considering the research strategy for SHSC, REF and development of new research centres. It is likely we will be able to work cross school, CMC being part of maternal healthcare and thus nursing.

We aim to transition from researchers to an independent CMC research group, generating research vision with an adjacent agenda. Establishing a sustainable research vision will be guided by feedback from peers and colleagues, incorporating others’ opinions on logic and feasibility, drawing on a comprehensive knowledge reservoir.

The possible futures of midwife-led continuity of care
2023 - 2024

Description Public Engagement Fund 2023/24:

There is a sharp increase on future thinking because of increasing concerns about the future. In maternity services, midwife-led care continuity care (MLCC) is a topic facing an uncertain future, although being the main recommendation in the Best Start – a future orientated 5-year plan for the future of Scottish maternity services. Earlier research activities (from applicant) using state-of-the-art methodologies (logics scenario development methods, future wheel) materialised into four possible futures of MLCC (https://youtu.be/NnDsIyp6D1Y). These MLCC futures, structured as scenarios/narratives, can serve as a medium to think about which futures are desired, should be pursued or to be avoided in terms of maternity services from a service user perspective. The narratives/scenarios are written from a stakeholders’ perspectives, based on evidence that derived from research, identifying the trends and macro developments that influence the future of MLCC.

In terms of wider communication, considering (literacy)inclusivity, the written scenarios would benefit to be transformed to audio-visual products. Before transforming the written/verbal scenarios to audio-visual products, the scenarios and appropriate audio-visual materials need to be explored and validated by the audience of interest. Reflection on, and critical discussion about the scenarios’ content, the impact of the futuristic meaning of each scenario and its linguistics features are needed as well as which visual materials are preferred, resonating with the pregnant/childbirth population. Taking these steps will contribute to a widespread MLCC communication product that critically informs the public when thinking about maternity care, specifically those who mainly benefit from MLCC - pregnant/childbirth people.

Description This project provides a method of anonymously exploring and portraying the perceptions of women who currently use or have used Scottish maternity services since 2017 - to help capturing their experiential reality of midwife-led continuity of care. The project uses a user-centred experienced-based co-design approach.

There is a sharp increase on future thinking because of increasing concerns about the future – four scenarios portraying possible futures of midwife-led care will be validated during this public engagement project.

Description The project is a result of collaboration between the School of Arts and Creative Industries (SACI) and the School of Health and Social Sciences (SHSC) (Interdisciplinary PE fund 2021-2022) using a user-centred experienced-based co-design approach. This project provides a method of anonymously exploring and portraying the experiences of women who are mothers to help capturing their experiential reality and knowledge in the everyday world of childbirth.

During pregnancy women and their partners have ideas and wishes about childbirth, which might differ from the reality of birth or the decisions that are taken during birth. Capturing women’s experiential reality of birth offers a platform for childbirth to be portrayed as a unique and important life-event in a woman's life. Concrete lively details in personal stories draw attention and enable others to learn about and visualise the event of childbirth. A visual archive keeps the experiences alive to be shared with the wider society.

Description This body of work is part of the Continuity of Midwifery Care (CMC) research network which aims to unite academia and practice in Scotland and boost the evidence and conversations regarding CMC. As a group we aim to draw on knowledge and expertise within our network to establish leadership in CMC.

Current projects include:

Evaluation of Continuity of midwifery care in Scotland: A nested case-control study.
Aims: To evaluate the outcomes of midwife-led continuity of care by examining the differences in outcomes between those who received midwife-led continuity of care and those who did not (care-as –usual) in NHS Lothian.

Midwife-led continuity models versus other models of care for childbearing women: Updated systematic review and meta-analysis.
Aims: To compare the effects of midwife-led continuity models of care with other models of care for childbearing women and their infants adding to the existent body of knowledge in the Cochrane review (Sandal et al., 2016).

Development and evaluation of a Continuity of Midwifery Care model for pregnant people with Complex Social Factors.
Aims: To develop and evaluate a new service within NHS Lothian to provide continuity of midwifery care in order to improve maternal and neonatal outcomes.
