CSO Clinical Academic Fellowship Scheme

Description Successful Chief Scientist Office, Scottish Government Health and Social Care directorates (CSO Clinical Academic Fellowship Scheme) grant application (£190,259).

Midwifery shortages have been a significant problem for some time. However, in recent years, reports relating to short staffing and a potential impact on safety for women have been increasing. Maternity reviews now frequently implicate staffing as a contributor to adverse outcomes for women and babies.
While there is a need to invest in more staff, midwives are leaving at such a rate that this alone is not an achievable way to attain or maintain a sustainable midwifery workforce. The need to prevent midwives leaving is therefore an increasing focus of concern.
While there are some survey studies that explore why midwives stay or leave, there is a significant gap in the research in terms of: research from the United Kingdom; in-depth exploration of the factors that influence retention; and solution focused research that aims to prevent attrition. This is surprising given the well-established nature of this problem, and its impact on outcomes, experiences, and safety for service users.
This research will undertake an in-depth exploration of the factors that influence retention to develop a multi-pronged retention strategy.

Aim:
To develop a strategy to encourage midwives to remain in clinical practice

Objectives:
•To explore factors that influence the decision to leave clinical practice
•To develop a retention strategy based on this exploration
•To explore whether the strategy appears acceptable and feasible in practice
•To plan for implementation and evaluation of the strategy

Stakeholder groups will inform the development of the research throughout. They will develop the retention strategy and identify key uncertainties at each of the following phases.

Phase 1: Literature review and systems mapping.

Phase 2: Health Board level analysis.
One Scottish Health Board has been identified for in-depth study of the factors that influence retention.

Phase 3: Strategy acceptability and feasibility
Exploration of the acceptability and feasibility of the strategy, as well as changeability of the system, to refine the strategy for use in practice.

Phase 4: Implementation plan for post-doctoral phase
With stakeholder groups, a will plan be developed for a post-doctoral implementation and process evaluation study.

Developing a midwifery continuity of care research group

Description Research Culture Fund 2023/24:

The key component of the Scottish’s Best Start Government 2017 maternity plan is Continuity of Midwife Care (CMC). Although the topic of CMC being embedded in the midwifery curriculum, we would like to shift focus to a research agenda within the midwifery subject group. We are a group of active researchers, in various stages of research careers, with a shared interest and passion for CMC with individual publication output but have little in the way of a shared CMC strategy or plan. This project offers an opportunity to instigate a group leading CMC research and tie this project to the wider school and university agendas considering the research strategy for SHSC, REF and development of new research centres. It is likely we will be able to work cross school, CMC being part of maternal healthcare and thus nursing.

We aim to transition from researchers to an independent CMC research group, generating research vision with an adjacent agenda. Establishing a sustainable research vision will be guided by feedback from peers and colleagues, incorporating others’ opinions on logic and feasibility, drawing on a comprehensive knowledge reservoir.

The possible futures of midwife-led continuity of care

Description Public Engagement Fund 2023/24:

There is a sharp increase on future thinking because of increasing concerns about the future. In maternity services, midwife-led care continuity care (MLCC) is a topic facing an uncertain future, although being the main recommendation in the Best Start – a future orientated 5-year plan for the future of Scottish maternity services. Earlier research activities (from applicant) using state-of-the-art methodologies (logics scenario development methods, future wheel) materialised into four possible futures of MLCC (https://youtu.be/NnDsIyp6D1Y). These MLCC futures, structured as scenarios/narratives, can serve as a medium to think about which futures are desired, should be pursued or to be avoided in terms of maternity services from a service user perspective. The narratives/scenarios are written from a stakeholders’ perspectives, based on evidence that derived from research, identifying the trends and macro developments that influence the future of MLCC.

In terms of wider communication, considering (literacy)inclusivity, the written scenarios would benefit to be transformed to audio-visual products. Before transforming the written/verbal scenarios to audio-visual products, the scenarios and appropriate audio-visual materials need to be explored and validated by the audience of interest. Reflection on, and critical discussion about the scenarios’ content, the impact of the futuristic meaning of each scenario and its linguistics features are needed as well as which visual materials are preferred, resonating with the pregnant/childbirth population. Taking these steps will contribute to a widespread MLCC communication product that critically informs the public when thinking about maternity care, specifically those who mainly benefit from MLCC - pregnant/childbirth people.

Description Researcher Development Fund 2022-2023:

The key component of the Scottish’s National Health Service Best Start Government 2017 maternity plan is Continuity of Midwife Care (CMC). Despite the evidence that this model of care is superior in terms of health outcomes for mothers and their families, resources should be provided to affect reasoning and beliefs of midwives to actively embrace the operational implementation of CMC. The ENU midwifery subject group and NHS Lothian researchers have published several papers about CMC, recommending that ground-up change needs to be actively supported. Knowledge is one of the facilitators for the ongoing implementation of the CMC model by the (future) midwifery workforce. The flow and dissemination of knowledge should be offered to (student)midwives on a regular basis, short messages and easy to comprehend including a pragmatic aspect – applied evidence. This can be achieved by an online freely available resource that is easily accessible for (student)midwives to find important and relevant knowledge about CMC. A newsletter is user-friendly and effective wat to directly communicate with the target audience. (ENU) Researchers who like to share their knowledge, can use the newsletter to disseminate the latest (published) evidence.
