Pressure ulcer grading tools: how reliable are they?

Abstract
Reports have indicated that 45\% of some hospital populations have a degree of tissue damage as a result of pressure. The reliability and validity of pressure ulcer risk assessment tools have been discussed in the literature, but little has been written about pressure-damage grading tools, the development of which does not appear to have been subjected to rigorous research. Despite this, these grading tools are in everyday use. Additionally, some knowledge is required to interpret them, yet nurses are not always able to identify the underlying tissues exposed.
