Abstract
To protect against accidents and their consequences, companies may take one of several courses of action. They can choose to do nothing, and live with an 'acceptable risk', or they may invest heavily in training, technology, preventive maintenance and damage limitation exercises. There are several courses of action in between these two, but at what point does a precaution become one too many?
