P107 Non completers in pulmonary rehabilitation – are they activated?

Abstract

Background: Identifying potential participants who do not complete pulmonary rehabilitation (PR) programmes is a continuing problem. This study looked at the usefulness of the Patient Activation Measure tool to predict non-attendance.

Methods: A casenote review identified 28 participants who started but did not complete PR. All had attended an assessment session where some basic information and one PAM questionnaire had been completed.

Results: 28 participants (15 F, 13 M), with a mean age was 67.93 (±8.875), most (n=17, 61%) had a deprivation score of 3 or lower (1=most deprived), as well as a mean MRC score of 3.3 (±0.793) and a BMI of 28.9 (±8.28). Most were living with family (n=17, 63%). Mean FEV1%pred (n=14) was 49.76 (±14.975). Most had a mean of less than 1 (0.68±1.282) admission and 3 exacerbations (3.22±2.283) in the previous year. The mean adjusted score for PAM was 56.95 (±14.65, PAM Level 3). In this level patients are beginning to self-manage their own condition and take responsibility for their own health. A large percentage of the participants did not give a reason for non-attendance (18, 64.3%), of those who did, they cited significant life changes (n=1), injury or illness (n=5), previous completion (n=2) or deferred or requested discharge from the programme (n=2).

There were no significant associations between adjusted PAM score and MRC Score, BMI, oxygen level, FEV1 percent predicted or living situation. There was a negative correlation between deprivation score and activated score (Pearson correlation=−0.487, p=0.010). Similarly there was a negative correlation between admission number and adjusted PAM score (Pearson correlation=−0.463, p=0.023).

Conclusions: Baseline PAM scores were higher than expected with a mean of 56.95, i.e. level 3. Two correlations have shown that both number of admissions and deprivation score were significantly negatively associated with adjusted PAM score, i.e. those with a lower number of admissions had a higher adjusted PAM score, and those with a lower deprivation score had a higher adjusted PAM score. Further research is needed to identify those who may not complete PR.
